How Services Are Decided and Requesting Support
Home and Community Care services are based on each person’s assessed needs. We understand every person’s health, home situation, family supports, and safety needs are different, and care plans may not look the same for everyone.
Fair care does not always mean that everyone receives the same service. It means that each person is supported in a way that is appropriate to their needs and circumstances.
We will work with you to understand your situation, explain the options, and identify the most appropriate next steps.
You can contact the Home and Community Care team directly. A family member, caregiver, hospital team, or health provider may also contact us on your behalf, with your knowledge and consent. After we receive a request, a member of the team will reach out to discuss your needs and arrange an assessment where appropriate.
